You can enable and customize a contact form in QuickTest to gather information from your participants.

To customize your contact form
- In the top-right corner of the Contact Form section, set the toggle switch to ON.

- To modify your contact form's settings, click the Pencil icon
in the top-right corner of the Contact Form section. - Enter a brief explanation for your participants about why you're collecting contact information in the Page Text entry field.
- Select the form fields you want to use in your contact form.
- Note: All contact form fields are on by default, but none are required.

After you’ve selected which form fields you want to use, you can select which form fields you would like to mark as required fields. We recommend requiring at least a first or last name, as all unnamed participants will appear as Anonymous in your participant list.

- If you want to include a link in your contact form that displays additional information about your company when a participant clicks it, complete the following steps:
- Set the Additional Company Information toggle switch to ON.
- In the Company Name field, enter your organization’s name.

In the Company Information field, enter the additional information you would like to have displayed to the participants. This can include contact information for your organization.

- When you're done, click Save.
- Note: If you deselect all form fields and click Save, your contact form will be turned off.
iPad region
Your iPad's configured region will affect phone number validation in the contact form. This is important, as customers who enter their region-specific phone number will receive an error message if your region settings have not been configured. Please ensure your iPad's region matches the region in which you will be testing.
To change Your iPad’s region:
- On your iPad, navigate to Settings > General > Language & Region > Region.
- Select your country from the list.
- Tap Continue.
- After setting your new region, verify that the date and time look the way you want them reflected in the test results.
Contact consent statement
Located directly beneath the Contact Form section, the contact consent statement will add a statement to your test that says, "By tapping the button below, you are consenting to be contacted by [Company Name]. You may unsubscribe at any time" along with an Agree & Continue button.

To enable the contact consent statement, set the toggle switch to ON.

- In the Company Name field, enter your company name.

In the Company Information field, enter your company's contact information.

Click Save to save your changes.
