When shipping and receiving your SHOEBOX products (especially new orders with a total cost of $2500 USD or greater) your organization will need to have a current Form 5106 on file with the United States Customs and Border Patrol (CBP) before the shipment can be processed.
Form 5106 is not a request from SHOEBOX specifically, but rather a requirement from the CBP for high-value orders to pass through US customs.
A CBP Form 5106 is required for all importer filings that are classified as formal entries. A formal entry is a type of import that is either intended for commercial purposes or valued at more than $2,500.
Even if your organization has submitted Form 5106 in the past, the CBP may still require an up-to-date form to be filed.
With regard to shipments of audiometers equal to or greater than $2500 USD, the CBP will always require an up-to-date Form 5106 to be on file.

Important: All customers must provide a tax ID number or EIN on Form 5106, even if you are tax-exempt. Contact your finance department if you are not sure what this is. Next, refer to the Form 5106 Guide for more information on how to fill out the form easily and efficiently. After completing and filing Form 5106, there may be additional taxes and duties that need to be paid based on your organization’s specific circumstances. Take note that like Form 5106, these fees are not tied to SHOEBOX. They are import requirements between your company and the CBP. |
