An individual patient's page contains several sections. What you’ll see is dependent on what tests and activities have been performed.
Only tests and activities (Audiogram, SRT/SDT, Speech Discrimination, and Questionnaires) from the most recent testing session will be displayed in full. Past tests and activities will be available for download in the Documents section. Summary reports and referral letters will be available for download by clicking on the Export button. Required action
The purpose of the Required Action section is to help manage the progress of patients when requesting reviews and retests.
To set a required action for a patient
- In the top-right corner of the Required Action section, click the Set Next Action button.
- Select an option from the dropdown list of available actions.
- Add a note to explain why the action is required.
- If a retest is required, check the Retest Required checkbox below the dropdown.
- Click Save Next Action.
- When the action has been completed, follow the steps outlined above to change the action. If no further action is required, set the action to None.
Patient demographics
This section contains information such as the patient's name, birth date, gender, contact information, and employee information. You can configure which information is displayed in this section from the Patient Records page. You can also lock the demographics for imported patients so that the information cannot be edited.
To configure which patient demographics are displayed

In the sidebar, hover over Settings, then click Patient Records under the System Setup heading.

In the Patient Demographics section, select or deselect which fields you would like to have displayed.
- In the top-right hand corner, click Save to save your changes.
To lock demographics for imported patients

In the sidebar, hover over Settings, then click Patient Records under the System Setup heading.

To prevent imported patient demographics from being edited, set the toggle switch to ON in the Lock Imported Demographics section.
- In the top-right hand corner, click Save to save your changes.
Projects
The Projects section displays a list of all projects the patient has been assigned to.
Analysis
This section contains questionnaire scores (if applicable), as well as Shift or Grade calculations.
Audiogram
This section contains the most recent audiogram results, which can be viewed in both audiogram view and table view.
Audiogram view
- Displays individual audiograms for each ear at full screen view.
- Displays a combined audiogram at mobile screen view.
- Does not include extended high frequencies.
- Overlays include Hearing Loss Grades, Speech Banana, and Established Baseline.
Table view
- Displays individual test data for each ear in separate tables.
- Includes extended high frequencies.
- Hint: Tests including extended high frequencies will default to table view.
- Overlays include Established Baseline.
To change the display options
- In the top-right corner of the Audiogram section, click the Display Options icon
.
- Select which display view you’d like to use.
- Select or deselect any additional overlays.

To configure which display options are shown by default for your organization

In the sidebar, hover over Settings, then click Patient Records under the System Setup heading.
- In the Audiogram Display Options section, select your preferred default view.

Select or deselect any additional overlays.
- In the top-right hand corner, click Save to save your changes.
Audiogram history
This section contains all past and present audiograms associated with the patient.
Imported audiograms are indicated with an arrow while SHOEBOX-created audiograms have no indication.
You’ll also see either AHL (Average Hearing Level) or PTA (Pure Tone Average) calculations (depending on your organization’s settings) for each audiogram, as well as a list of which frequencies are being used in the calculations.
Finally, clicking the three-dot icon on the right-hand side of each audiogram will open a sub-menu that allows you to perform the following four actions:
- View: View the audiogram.
- Delete: Permanently delete the audiogram.
- Download: Download the audiogram in PDF format.
- Location: Open a Google Maps window to show you the location of the audiogram's test site.
To mark an audiogram as a baseline
- Click the box in the LB (Left Baseline) or RB (Right Baseline) column of the test you wish to set as a baseline
- You will then be asked to confirm that you'd like to set or change a baseline in a popover window. To remove a baseline indication, click on the arrow in the LB or RB column beside the test you are changing.
To manually input a historical patient audiogram
- In the top-right corner of the Audiogram History section, click the New Audiogram button.
- A popup will be displayed with all necessary fields for adding audiogram data.
- For each frequency, you can input a number to indicate a threshold (for example, 70) or a number with a plus sign (+) to indicate a no response (for example, 90+)
- Leave any untested frequencies blank.
- If this is a baseline test, select the Mark as Baseline checkbox.
- When you’re done adding all audiogram data, click Save.
- Note: Audiograms cannot be edited, so ensure all data is correct before clicking Save.

The audiogram will now be displayed in the Audiogram History section as well as in the Documents section.
Audiograms voided by reviewers
If your organization is partnered with our audiological review network service, you will be able to see if an audiogram has been voided by your reviewer. Voided audiograms are marked with a strikethrough.
Questionnaire
This section contains any questionnaire answers given by the patient from the most recent testing session.
Documents
This section contains all test results and questionnaires from newest to oldest. You can view any test result by clicking on it.
Audiogram test results will be downloaded as a PDF, and all others will be displayed in your web browser. If you need to print any non-audiogram results, you can either take a screenshot (Print Screen key on windows, Shift+Command+4 on a Mac) or print the full web page from your browser.
