This article describes the shift rule settings that are used for organizations in clinical or research settings. For a description of the shift rules used for occupational health, see Program Setup I - Occupational Health Shift Rules.
You can find the settings for shift rules, scheduling rules, and automatic triage under Settings > Program Setup in the side menu of the Data Management Portal.
Shift rules
The Shift Rules page is where you can configure rules for shift/averaging calculations and assign them to projects. Shift reports and the automatic triage process rely on the correct configuration of these rules. Clicking the Pencil icon will allow you to configure rule settings.
The settings you can select in this section are the Shift Standard and the Averaging Label.
| Note: The Default shift rule applies to all patients in projects that do not have a shift rule configured, as well as any patients assigned to the Default project (unassigned). |
Shift standards
Shift standards are shift calculations or grading scales used to evaluate hearing changes. Here, you can set which calculations are used by your organization. These calculations are used for patient analysis, summary reports, and automatic triage.
Shift standards use the results of a patient's audiogram to automatically calculate things like recordable shifts in a patient's hearing ability, or measurable grades of hearing that are given to a patient based on their audiogram results.
During your testing, you may be required to use different calculation standards, based on your specific use case. Below are descriptions of the different calculation standards that apply to clinical and research settings.
CTCAE
Common Terminology Criteria for Adverse Events (CTCAE) is commonly used as the standard classification and severity grading scale for adverse drug events in cancer therapy. Many clinical trials have adopted this 5-point grading scale.
Select this if you would like to include a CTCAE grade on your patient's results.
Brock
The Brock grading scale is a 5-point scale specifically designed for pediatric oncology patients treated with cisplatin. This system is used to grade hearing loss progression from high to lower frequencies, considering the typical drug-induced configuration of high frequency loss. The grade is calculated for test frequencies from 1000 to 8000 Hz. Note that a grade of 0 does not necessarily indicate hearing within normal limits with this scale.
Select this if you would like to include a Brock grade on your patient's results.
Averaging label
When you select CTCAE or Brock as the calculation standard, you cannot select an averaging label, but you can select which frequencies are used for testing. The frequencies that you select here will be used to calculate the average.
Creating new rules
You can create additional program rules that differ from the default rule and assign them to specific projects. Take note that while you can create new rules, the available options are based on predefined standards, and you cannot customize their settings.
| Note: The Default shift rule applies to all patients in projects that do not have a shift rule configured, as well as any patients assigned to the Default project (unassigned). |
To create a new rule for clinical & research settings:
- Click on the New button in the top-right corner of the page.
- Give your new rule a title, assign it to a project(s), and select the shift standard and frequencies you would like the rule to use.
- Click Save to save your new rule.
Your new rule will then appear above the default rule. Rules can be edited at any time by clicking on the Pencil icon .
